Laguna Honda Hospital

Laguna Honda Hospital and Rehabilitation Center (LHH) is one of the sites for the transfer of SFGHTC patients.   LHH has been in operation since 1866 and is the largest single-site, municipally owned and operated skilled nursing care facility in the country.  It is a licensed acute care hospital with a distinct part that provides skilled nursing care and another part that provides rehabilitation care.  It is a part of the County Health Network that is owned and operated by the City and County of San Francisco’s Department of Public Health.  A multidisciplinary team consisting of professionals in nursing, medicine, social services, nutritional services, activity and recreation therapies, pharmacy, and rehabilitation therapies care for each resident. Laguna Honda Hospital provides a full range of treatment to the adult residents of San Francisco, who are disabled or chronically ill, including specialized care for those with wounds, head trauma, stroke, spinal cord injuries, orthopedic injuries, AIDS and dementia. The hospital also has a hospice program. 

In Home Support Services

This program provides attendant care in the home to assist the patient with activities of daily living such as bathing, shopping, cooking, cleaning and laundry. The services can be provided by either a family member or an employee of the agency.

Health at Home

Home services provided by Health at Home are available to patients who are home bound. Since the patient is not well enough to leave their home but too well to be in a facility, services are provided to the patient in their own home. These include: temporary nursing services; physical, occupation, and speech therapies; and social work services.

Adult Day Health

Adult Day Health is a form of adult day care where your loved one can spend five hours a day being looked after in a center. Meals, activities, assistance with medication, and therapies are all provided. Patients typically are transported by van and this program allows disabled patients to remain in the home even when family members are working during the day.

Outpatient Rehabilitation

Outpatient rehabilitation services including physical, occupational and speech/language therapy are provided at San Francisco General Hospital and Trauma Center and Laguna Honda Hospital. Patients must be well enough to tolerate transportation to and from the hospital or can be escorted by family. These services are ordered by the primary care provider, specialty care provider or arranged at the time of discharge from rehabilitation.
